Includes radiographs, soft tissue, prevention (reducing sugar, stop smoking, alcohol), instruction and removal of plaque retentive factors, fluoride varnish, fissure sealants (for enamel caries)

Based on recall interval to NICE guidance, (contains the same as New Patient exam)

Includes up to a maximum of 4 teeth. (permanent filling, temp crowns, and extractions)

Between 5to 8 teeth (permanent filling, temp crowns and extractions. Composite for anterior teeth, amalgam or amalgam alternatives for posterior)

To include plaque score, oral hygiene instruction, 6-point pocket chart, removal of plaque retentive factors and pocket debridement. (up to 2 packages per year).

Does not include lab fees, these are paid directly to the lab. For new dentures any easing or adjustments are covered free of charge for 2 years. On immediate dentures its only 2 months.

Lost dentures are not covered under a 2-year warranty.

For patients who have 7 or more carious teeth, and at least 2 of the teeth have caries into the pulp. Includes extractions, oral health prevention, GI filling, pulp extirpation or removal of plaque.

Up to 2 teeth and includes permanent restoration. (temp crowns and permanent filling are included in the package).

ONLY AVAILABLE FOR NON-ACTIVE PATIENTS FROM THE LHB.

Should consist of permanent definitive treatment including restorations, 12-month warranty. If problems continue with the same tooth the patient can contact the practice themselves with the 12 months.

If further treatment is needed the practice should either take the patient on as a new patient (by contacting the LHB), or do an onward referral if needed, or finally you would advise the patient to register on the DAP.

Denture repair, ease or reline outside of the 2-year warranty, study models, repair or recement crown/bridge, removal of sutures, pericoronitis, ANUG, arrest of haemorrhage, dry socket, splints, trauma

These can be booked through the DAP, or if they are children of existing Active NHS Patients.
